Community Support Membership Plan:
Finding medical transportation can be stressful and expensive no matter the situation. North Star Critical Care wants to help ease the burden and help you and your family be prepared for a medical event with our Membership Plan.
Membership includes:
- 2 home safety inspections per membership year
- Cost reduction for members who have met their required deductible (Medicare rules apply)
- Membership extends to most members of the household
- Spouse
- Dependent under 21 years of age
- Dependant under 25 if enrolled in college
- Dependent that is a resident of a skilled nursing facility
- Hands only CPR course
- Complementary First Aid Kit
- Invitations to community health and wellness events
- Membership Card
How it works:
While membership is not insurance, it helps you feel at ease by preparing your household for a medical event and protectiong you from potentially high out-of-pocket expenses, such as copays or coinsurance for medically necessaty transports that may not be covered by your insurance policy, Medicare part B, or other health benefits payor.
The Membership Plan is effective the date that North Star Critical Care receives a completed application and Membership Fee. Membership effective dates cannot be retroactive.
Membership includes only transport by ground ambulance provided by North Star Critical Care. Please be sure to request us if you do not reach out to us directly.
Simply complete the form and these benifits will begin once payment is processed. Benefits do notretroact. Insurance discounts do not apply to Medicaid recipients.
Emergency medical transports are based on medical necessity and transportation of patients to the closest, medcailly appropriate facility. Availability of services cannot always be gauranteed due to weather conditions, geographic restaints, limitations due to patient weight, or commitment to another patient. Provider will accept payment from insurance as payment in full; with the exception of the member's deductible, as Medicare does not allow the waiver of insurance deductibles.
Membership fees are non-refunadable. Provider is not an insurance company - Provider's membership program is not an insurance policy and cannot be considered as a secondary insurance coverage or supplement to any insurance coverage.

North Star Critical Care will never deny emergency medical care or transportation based on insurance status or ability to pay.
